| Cost: | $65 per participant before August 1st, 2013 $75 per participant after August 1st, 2013 |

| Note: | Player evaluations will be used to rate all player's abilities and will allow us to create even and fair teams. Please have player at evaluation is possible. If your player does not attend evaluation, they will be placed on a team by program coordinator. |
